Procedure for Borrowing Accelerometers from the CLOCC Library

1. The interested party must contact CLOCC to request the accelerometers.

2. CLOCC will send out the Accelerometer Fact Sheet and Borrowing Form to the interested party.

3. Once the form has been completed and returned, CLOCC will forward a copy of the form to ActiGraph and will provide the interested party with ActiGraph’s contact details.

4. ActiGraph will make arrangements with each party to sign a User Agreement taking all responsibility for any damages to/loss of the units.

5. All transportation of accelerometers and communication will occur directly between the interested party and ActiGraph, unless otherwise specified by CLOCC (eg, if 2 parties need the devices at the same time and CLOCC has the interface reader).

6. It is the interested party’s responsibility to arrange and pay for shipping of the units back to ActiGraph.

7. It is the interested party’s responsibility to inform CLOCC of the actual date of borrowing and return of the units and if there is any change required from the dates agreed upon.
