Blueprint Survey (January-June 2015)

CLOCC’s Blueprint for Accelerating Progress in Childhood Obesity Prevention in Chicago: The Next Decade lays out goals and recommendations for action in six focus areas: Food and Beverage Access, Physical Activity and the Built Environment, Schools, Early Childhood, Business Sector and Industry Practices, and Health Promotion and Public Education. It is meant to be a guide for childhood obesity prevention work in Chicago with the idea that all of our collective work as a consortium will help to address these goals.

We invite your organization to participate CLOCCin a short survey on your work from January 1 through June 30, 2015, related to the goals, objectives, and strategies in the Blueprint. We are interested in all activity related to this work and would like to hear from all organizations – even if you don’t provide direct services or run programs. We’re interested in research, policy initiatives, media and marketing, community initiatives, etc. – anything you think advances the goals and objectives in the Blueprint. The survey will remain open for your participation until Friday, August 14, 2015. Your response will help us gather information about the progress we, as a consortium, are making toward the Blueprint goals.

A bit about the survey:

  • We are trying to capture all work related to the goals, objectives, and strategies in the Blueprint, so even if you don’t provide direct services or run programs we would like to hear from you.
  • We would like to hear from anyone in your organization with information on the childhood obesity prevention activities in which your organization is involved.
  • The survey is set up by focus area. If there is a focus area your organization is not working in, you will be able to skip that section and only answer questions about the focus areas in which your organization is involved.
  • Within each focus area section, we will ask about your involvement in each objective mentioned in the Blueprint and then provide space for you to provide more detail about the specific work you are doing.

The survey must be completed in one sitting, so you may want to take a look at the Blueprint before you begin in order to start thinking about how your work fits in with the focus areas and goals/objectives in the Blueprint.

The information you share helps us to track and report Chicago’s progress in childhood obesity prevention. We will also use the survey results to identify partners to highlight throughout the year for the excellent work they are doing to advance the Blueprint goals and objectives.
